Regardless of etiology, the hallmark is inappropriate aldosterone activity relative to sodium status and renin physiology.

Screening commonly uses the aldosterone-to-renin ratio (ARR) measured under standardized conditions. Interpretation requires careful attention to medications and physiologic variables that can distort results. For example, mineralocorticoid receptor antagonists, diuretics, beta blockers, and other agents can shift renin and aldosterone concentrations; hypokalemia can also suppress aldosterone in some contexts. Many protocols recommend correcting potassium, reviewing medication timing, and adjusting interacting therapies when feasible, ideally under specialist guidance.

Confirmatory testing may follow a positive ARR, including saline infusion testing, oral sodium loading, fludrocortisone suppression, or captopril challenge. The goal is to demonstrate autonomous aldosterone secretion that fails to suppress appropriately. After biochemical confirmation, subtype evaluation distinguishes unilateral from bilateral disease. Imaging with adrenal CT can identify structural lesions, but it may miss microadenomas or detect incidentalomas. Therefore, adrenal venous sampling is often used in surgical candidates to determine unilateral aldosterone production more accurately.

Treatment aligns with etiology. For unilateral primary aldosteronism, adrenalectomy can offer the potential for cure or marked improvement, with benefits including reduced cardiovascular risk over time. For bilateral disease or when surgery is not preferred, long-term pharmacotherapy with mineralocorticoid receptor antagonists is standard. Spironolactone and eplerenone block aldosterone’s receptor-mediated effects, reducing sodium retention and mitigating aldosterone-related vascular and renal injury. Dosing is titrated to blood pressure response and electrolyte stability, with monitoring for hyperkalemia and renal function changes.

Beyond disease-specific therapy, patients require comprehensive cardiovascular risk management: lifestyle modification, adherence support, secondary cause review, and appropriate selection of complementary antihypertensive agents. Particularly in aldosterone-mediated hypertension, optimizing diuretic strategy and ensuring adequate blockade of mineralocorticoid signaling can be decisive.

In summary, aldosterone excess represents a biologically plausible and potentially underrecognized driver of hypertension, especially in those with resistant disease. Recognizing the role of aldosterone helps clinicians move from a one-size-fits-all approach toward mechanism-based care, using ARR screening, confirmatory testing, and targeted treatment to improve blood pressure control and reduce long-term cardiovascular and renal complications.
